Any student in Middle School who missed a section or more of the ERB CTP5 testing will have their tests scored without the missed section(s), unless it is the parents’ preference to have the student miss classes to make up the missed sections. Please fill out this form for your child if he or she missed any section(s) of the ERB CTP5 that were administered the week of November 6th and you would like your child to miss classes on Friday, December 1 to complete the portion(s) he or she missed. If you do not fill out this form, your child’s ERB CTP 5 will be scored without those sections included in his or her score report. ERB CTP 5 does not have a “composite” score; your family would simply get results for the sections the student was present to take.
