Each school year, we ask parents to complete forms which allow Providence Academy to receive funds supported by your tax dollars which are otherwise directed toward public schools. Providence Academy uses these funds from your tax dollars for the purchase of textbooks and to support the health services department and guidance services for the benefit of Providence students.

NEW THIS YEAR—To simplify the processing of these forms, simply click this link to complete the three-question Google form electronically. We appreciate your prompt attention.

By completing the google form for your family, you approve PA receiving funds for the following;

For each K-12 student: Textbook, Providence Academy receives tax reimbursement funds from the Wayzata school district to help cover text book costs.

For each K-12 student: Health services helps offset the cost of having a full-time Registered Nurse at Providence and provides funding for health supplies

For each 6-12 student, Counseling Services, through the Wayzata school district, Providence receives reimbursement for the counselors on staff in the middle and upper schools.

Please note that the reimbursement money comes from your tax dollars and was designated for books and services for your student(s). Additionally, for Providence to receive annual funds, a completed form is required from every family for every student every school year.
