Application for Admission: complete and return to Providence Academy.
	Teacher Referral: submit to child's current teacher(s). Teacher will return the referral to Providence Academy.
	Request for Student Records: submit to child's current school office. The school will forward the records to Providence Academy
	Candidate Questionnaire (for students applying for admission to Grades 6-12 only): attach to Application for Admission.
	Transcripts (from first semester of the current academic school year): attach to Application for Admission only if school does not release them.
	Check (for non-refundable $100.00 application and testing fee): attach to Application for Admission.

Please note: a completed Application for Admission may be submitted as early as desired to hold a place for your child at Providence Academy. The $100.00 application and testing fee MUST accompany the Application for Admission.
